IMPORTANT INFORMATION CREDENTIAL RENEWAL 2015
Please remember to check the following information before you send your Renewal Form:
Verify that:

All questions are answered and no blank spaces are left.

Remember to mail your District and General Council renewal form to our office by the deadline.

Remember to mail your District and General Council dues in TWO SEPERATE checks.

Do not forget to include your Church dues (if you are a Pastor).
Please mail your renewal forms on time. This will avoid any delay and unnecessary problems in
the process of your application.
Annual Dues for General Council
Do not forget to include your check made out to the General Council according to the level of credential you have.
Ordained $240.00 - Licensed $180.00 - Certified $ 90.00.
MINISTER DUES
Please remember that your Minister Dues need to be up-to-date in order for your credentials to be renewed. The following table shows the monthly and annually dues that needs to be send according to your credentials

IMPORTANT DATES FOR RENEWALS
A. FEES FOR CREDENTIAL RENEWALS

1- ON TIME RENEWAL $ 25. 00 if you mail by October 31st, 2015 (Please make your check payable to Southern Pacific District.)

2- LATE RENEWAL $75.00 If you mail “ Between January 1-15, 2015. All late renewals must pay penalty fees. (Please make two separate checks one for $25 payable to Southern Pacific District and other for $25 Payable to General Council.)

3- LAPSED RENEWAL $175 ” If you did not mail your credential before January 15, as of January 16, 2015 your credentials are Lapsed and you need to call the office to reinstate credentials. Avoid lapsing your credentials because reinstating would require a background report as well as a new credit report. (You would have to make two separate checks, one of $125 for Southern Pacific District and the other of $50 for General Council.)

B. DEADLINE DATES

Your General Council and District applications need to be in our District office with the date postmarked ON or before October 31st, 2013.

All applications received after October 31st, 2015 that does NOT have the postal stamp before or of Ocober 31st, 2015 Has to pay the late fee of $75 TOTAL.

Any form received after of January 16, 2015 will have to pay $175 and must reinstate your credentials.
C. LAPSED CREDENTIALS:
Any application received in our office by January 16, 2015 WILL NOT BE ACCEPTED. You need
to call our office and request a reinstatement application.
If you do not request to renew your credentials before the deadline your credential will lapse.
D. “ACMR” FORM (Only for Pastors):
General Council will mail the “ACMR” directly to your Church. You must complete and return
one copy to General Council and another copy to our District Office by October 31, 2015.
If you do not receive an ACMR form in the mail, you have the option to fill one out on-line on the General Council’s web page: www.Gensec.ag.org. If you do not have accesses to the Internet please call Yanina at our office and she will help you to obtain this form.
It is very important that you return this form! If after 3 years you have not answered this annual questionnaire, the General Council office will contact our office to find out why you have not responded.
PASTOR, it is very important that you take your time to fill out this form. It is a document that will help guide your church towards the future. It also helps the National Office see the progress of the Hispanic churches in a national level.
E. HOUSE ALLOWANCE FORM
Attached you will find the 2014 Housing Allowance form. This Form is for your records and taxes purpose. You don not need to mail it back to our office.
